St. Anthony’s Clinic is seeking a part-time (0.2 FTE) Nurse Practitioner (NP) to provide outreach service to adults and children who are impacted by poverty, colonization, stigma, and homelessness. Providing accessible primary episodic care services that not only address immediate medical needs but also actively work towards decreasing barriers and social determinants of health in accessing care. The NP will work with other members of an interdisciplinary team, such as family practitioners, MOAs, RNs, and Mental Health professionals providing low-barrier episodic all-ages primary care. Site Information The Sooke Food bank serves on average 96 families per week, and an average of 78 children. The site has many volunteers and social services on-site and provides supports in an outdoor setting. Hummingbird Place in Sooke, provides support for people over 18 who are homeless or at risk of homelessness in need of immediate basic necessities such as food, clothing, outdoor living supplies and / or have self-identified as engaging in substance use, are having challenges accessing housing or have mental health concerns. A renovated clinical space is nearly complete. St. Anthony’s Primary Care Center provides the operational management of the outreach service. St. Anthony’s is Nurse Practitioner Clinic, caring for clients of all ages and complexities. St. Anthony’s is in the same building but separate from the Westshore Urgent Care center. Start Date As soon as possible. Hours of Work The ideal candidate can provide outreach services four Thursday’s from -hrs each month.
