job description – SOCIAL WORKER Island Sexual Health Community Health Centre (ISH) is seeking a Social Worker to work collectively with an interdisciplinary health care team at Island Sexual Health. Under the direction of the executive director the social worker is responsible for the management and development of the social work program, providing services to meet the emotional and social needs of the primary care patients in this practice by addressing the social determinants of health. ISH is a Community Health Centre (CHC) that provides wrap-around sexual health care, longitudinal care for priority populations, and allied health care. Utilizing a team-based care model the social worker will work closely with Family Practitioners, NP’s, a Mental Health Worker and RN team. Qualifications ·        Registered Social Worker, BSW ·        Minimum 2 years experience working with populations who are structurally vulnerable and/or who are marginalised by societal norms and systems. Knowledge, Skills, and Abilities Ability and interest to work within a team-based care model. Ability to use and exercise sound judgment. Ability to recognize patient and community needs and develop appropriate resources to meet social and emotional needs. Ability to make and implement decision, balancing organizational and program goals with available resources. Ability to work as a team member. Knowledge of community and public health resources. Demonstrated effective oral and written communication skills. Demonstrated organizational skills and ability to establish priorities and organize workload accordingly. Experience working with populations that are gender and sex diverse (2S/LGBTQIIA+), experiencing mental health and substance use challenges, with histories of trauma, and/or have structural barriers to health. ·        Knowledge of key components of cultural competency and awareness of differences, attitudes, and beliefs that would allow one to work across cultures effectively and appropriately. ·        Knowledge of Social determinants of health and related disparities. ·        Knowledge of current and ongoing history of social work in colonial processes and ability to balance a critical lens and anti colonial position to their work. ·        Experience working with under housed and structurally vulnerable populations and housing system navigation (preferred). General Expectations ·        Works on-site and available for collaboration for patient concerns/challenges that arise during the day. ·        Responds to MH crises, MH crisis follow-ups. ·        collaborates with the multidisciplinary clinical team in the development and monitoring of an individualized interdisciplinary plan of care for attached patients. ·         Refers to and consults with appropriate medical professionals for co-management of patients with complex medical and social needs. ·        Provides informal counseling as needed for acute situational stressors. ·        Connects patients to the most appropriate resources in community. ·        Collaborates with the interdisciplinary team on the implementation of processes and tactics that support a team-based care model. ·        Coordinates interdisciplinary meetings where MDs, RNs, CHW collaborate on care planning to best support patients (and each other). ·        Case management. ·        Available for in the moment on-site consults with clinicians Island Sexual Health Community Health Centre encourage applications from Indigenous persons, persons of colour, persons with disabilities, persons with experience of sex work and persons representing diverse genders and sexualities.
