Home and Community Care Support Services Erie St. Clair is committed to supporting healthcare in a manner that is consistent with patient and family centred care. This position understands the importance of quality and safety and requires a high degree of attention to detail and excellent time management skills. What will you do? Working in conjunction with the Patient Services Manager, the Mental Health and Addictions Nurse (RN/BScN), will be working with district school boards to advise educators, develop individual care plans in collaboration with other professionals and assist boards in developing strategies to address mental health and addiction needs. Extensive travel for this position is required between all sites of the Home and Community Care Support Services Erie St. Clair area. Responsibilities will include: assisting school boards in recognizing and responding to student mental health and addiction issues; providing services and support to students with mild to complex mental health and substance abuse issues; developing plans for clients with mental health and addiction needs including the transition of students back to school from hospitalization; medication management and providing support or intervention for issues such as self-harm or treatment refusal. The Mental Health and Addictions Nurse will also play a key role in helping students and/or parents’ access services such as family health and/or addictions agencies. The position will be located within the Erie St. Clair Region. Proficiency in both official languages is required. NATURE AND SCOPE:Patient Service DeliveryAdvise educators on potential side-effects of different classes of medicationsProvide medical consultation to educators regarding issues such as medication management for individual students, particularly those with complex medical conditions concurrent with mental illness or addictionsLiaise with children’s mental health agencies and primary care practitioners as requiredProvide support and/or intervention in complex issues such as refusal to attend treatment, self-harm, suicide, or violent behaviourSupport educators to meet the complex medical and mental health needs of students who require extra supports for health and/or safety concerns of self and/or othersProvide the support of a health professional in helping students and/or parent(s) to access services such as primary health care or a children’s mental health centre and provide education about options for treatment. Provide essential health related advice and support to the Mental Health leaders in developing comprehensive board plans to address a collaborative response to the mental health needs of students. Act as a spokesperson as required, and interpret the role of Home and Community Care Support Services to patients, health care professionals and to the public. Ensure positive public relations and effective co-ordination of services through ongoing liaison and participation on internal and external committees.Participate in the orientation of new staff, educators and students within district school boards.Assesses for and promotes a safe environment for patients, caregivers, family members, and staff. Adhere to health and safety policies/ practices developed and implemented by Home and Community Support Services Erie St. Clair.Participates in establishing, maintaining, and monitoring standards for the Care Coordination. This includes committee work and active participation and contribution to quality initiatives.What must you have? Regulated health care professional holding current registration with a regulated college in Ontario; appropriate university degree or a suitable combination of education and direct experience.Must be fluent in French and English - both written and verbal skillsCurrent registration as an RN/BScN in good standing with the College of Nurses of Ontario2-3 years of relevant experience required, experience in providing direct clinical mental health and/or addictions services for children and youth preferredSound knowledge of the Ontario health care system; the role of Care Coordinator as assessor and health planner, all relevant legislation and available local community resourcesSolid ability to use MS Office applications (., Word, Excel, Outlook, PowerPoint, and internet research skills and computer navigationAbility to analyze information, problem-solve and make good decisionsAccountable for own actions and decisions, making decisions within the scope of the position and referring issues/problems/events to the manager as requiredSolid documentation skills (clear, thorough, accurate and timely)Self-directed with the ability to organize, prioritize and multi-taskFlexible, adaptable and responsive to changeDetail orientedEffective interpersonal and communications skills and able to adapt communication style to suit different audiences of all ages and backgroundsAble to lead and facilitate community education sessionsCourteous and respectful in all interactionsAbility to establish and maintain a wide range of contacts with professionals and organizations within the communitySolid effective listening and facilitation skillsAbility to maintain confidential informationEmpathy to sensitive issuesWe have a mandatory COVID-19 vaccination policy.
