Purpose

The Community Specialist will report to the Program Supervisor. This role is front-line and will be responsible for acting as an ally and ambassador, working front-line and creating a safe, caring and community-enriched life for the individuals/families who hire us as a liaison between Program Supervisors and the Family. Thoughtful program planning and implementation of pre-determined goals (i.e. ISP or person-centered goals) and supports participants to connect with their communities - establishing authentic relationships and creating meaningful experiences. The conscientious In-home Community Specialist will act with integrity, be open to growth, live our purpose, cultivate teamwork, and put forth an extraordinary effort to empower people with special needs and their families to grow, succeed, and belong.

Key Functions

  • Provide purposeful support in a community-based setting (i.e participant’s home, group homes and/or community setting) in an engaging, fun and safe environment.
  • Provide personal care and services in all aspects of personal hygiene as required by the participant
  • Implement and utilize person
  • centered plans, programs, facilitate inclusion, and support models designed to support and assist participants in an autonomous role.
  • Utilize human rights approach to support and empower individuals to advocate on their own behalf.
  • Maintain timely, accurate and professional documents in accordance with AdaptAbilities standards and procedures.
  • Understand and comply with all emergency processes and risk management procedures

Qualifications :

  • Alignment with AdaptAbilities core purpose and core values
  • Clean class 5 driver’s license, minimum $2 million insurance disability and personal vehicle is considered an asset
  • University degree or diploma in Disability Services, Social Work or Human Services (or equivalent)
  • Acceptable combination of training and experience in the disability-serving sector is considered an asset
  • Ability to obtain clear Intervention Record Check
  • Ability to obtain clear Police Information Check and Intervention Record Check is required upon hire
  • Standard First Aid Certification CPR Level C and AED is required upon hire
  • Non-Violent Crisis Intervention, Abuse Prevention and Response Protocol are an asset
  • Experience supporting participants with complex support needs is an asset.

Hourly Rate : $16.82 to $21.31

  • To be eligible for benefits, you must be working a regularly scheduled 30 hours per week.
