Apply today to join our team!Vancouver Coastal Health is looking for a Social Worker Team Lead for the HIV/AIDS Program working out of Three Bridges Community Health Centre in Vancouver, BC. Apply today!As a Social Worker Team Lead – HIV/AIDS Program at Vancouver Coastal Health you will:Provide first line supervision and coordinate the day to day operations of the Case Management HIV program.Provide clinical direction, coordinates clients and assists the Manager with a variety of administrative activities including the transition to a new service delivery model to support intensive case management and ongoing modifications to workflow and structure to ensure efficient and optimal use of client care resources. Liaise with staff within the program and supports interdisciplinary practice, quality improvement initiatives and integration of services.Supervise social workers and other staff members, participates in staff selections, schedules/coordinates work assignments and priorities and facilitates workplace solutions.Act as a clinical resource for members and promotes interdisciplinary collaboration for concerns, continuity of care and community resource coordination.Participate in quality improvement activities, recommends and/or develops policies and procedures.Oversee the collection of statistical and performance data to ensure reporting requirements/submissions are met. Qualifications Education & ExperienceCurrent full registration with the British Columbia College of Social Workers.Master’s degree in Social Work from an accredited school of Social Work, plus five (5) years’ recent, related experience providing HIV/AIDS clinical services including one (1) year experience in a supervisory/leadership role, or an equivalent combination of education, training and experience.Valid BC Driver’s License and access to a personal vehicle for business-related purposes.Knowledge & AbilitiesComprehensive knowledge of Social Work theory, and practice within a client and family centred model.Comprehensive knowledge of the development stages in the individual and family life cycle of psychosocial issues that may occur in response to changing health and dependency needs.Broad knowledge of HIV/AIDS and communicable disease control.Broad knowledge of Mental Health and Addictions, and working with marginalized populations.General knowledge of Trauma Informed Practice and Harm Reduction approaches and Strengths-based care.Demonstrated ability to promote Indigenous Cultural Safety.Demonstrated ability to provide competent and culturally safety care in a variety of settings and with diverse populations.Demonstrated program development skills.Broad knowledge of other facilities and community resources.Broad knowledge of other health care disciplines and their role in client care.Broad knowledge of legislation and government policies and procedures that relate to client.Demonstrated ability to provide supervision, leadership, work direction and guidance.Ability to communicate effectively, both verbally and in writing with clients, families, team members, other groups and community agency personnel.Demonstrated listening and information seeking skills that promotes communication and lead to a co-operative approach to problem solving within an interdisciplinary setting.Demonstrated ability to establish workload priorities.Thorough knowledge of facilitation, mediation, and conflict resolution techniques, and demonstrated analytical and problem solving skills.Demonstrated case management, teaching/training, clinical support and consultation skills.Demonstrated ability to provide group leadership, facilitate staff professional development, facilitate change, and perform needs assessments.Demonstrated ability to develop and maintain collaborative working relationships, with a commitment to the development of a client-centered integrated community-based service delivery program and community involvement approach.Ability to operate related equipment including applicable software applications.Physical ability to perform the duties of the position.
