Manager of Care and Intake

OVERALL PURPOSE

To manage the SafeHope Home Care Team and overall maintenance workings of the residential program along with the intake processes and case management for residential participants.

GENERAL DUTIES

  • Consider and adhere to risk management and participant protection policies and procedures
  • Maintain proper procedures and paperwork, ensuring ethical and confidentiality guidelines
  • Participate in team approach to problem solving and goal setting

PRIMARY DUTIES

  • Receive and screen referrals and non-accepted participants to other services when possible
  • Interview, debrief and facilitate intake of possible participants, communicate and coordinate intake with Program Manager, ensuring all necessary forms are completed for each participant accepted
  • Case management for all clients in the residential program every other week, updating staff on any changes and liaise with therapists, doctors and addictions counsellors to ensure continuity of care
  • Maintain positive working relationships with community partners and agencies
  • Supervise & support Care Team, providing debrief meetings biweekly and performance evaluations
  • Be on a rotating back-up on-call shift with Care Staff Team Leads, including holidays.
  • Work collaboratively with other departments, Program Manager, Outreach, Admin to ensure programming goals are met for each participant
  • Along with the Care Team, manage the day-to-day operations of the Residential Home
  • Directly responsible for all training of new Care Staff, unless otherwise approved and to be completed when participants are not present in the home
  • Develop and review residential documents/forms and programs, ensuring all forms are completed and filed accurately

QUALIFICATIONS

  • Social Service Worker Degree/Diploma or Post-Secondary Education in a related Human Services field
  • At least 3 to 5 years experience working with youth and vulnerable populations
  • At least 3 to 5 years experience working at supporting survivors of abuse or human trafficking
  • First Aid/CPR Certification
  • Current Police Check with a vulnerable sector screening
  • Valid G licence with a clean driver’s abstract and liability insurance
  • ASIST training
  • NVCI or CPI Training an asset
  • Mental Health First Aid

Salary : $48,912.90-$55,000.00 per year
