JOB SUMMARY: The Intake and EMR Data Coordinator is primarily responsible for the intake process for children, youth and families, being referred to developmental services by telephone, fax or in person. Referrals sources include Physicians, Allied Health Professionals or families.

The Intake and EMR Data Coordinator will report directly to the Clinical Manager.

QUALIFICATIONS:

  • Persons of First Nations ancestry will be given preference (OHRC, Part II, Special Employment)
  • Minimum Grade 12 and/or post-secondary education in a health services field or equivalent working experience;
  • Diploma/Certificate/Degree in a health-related field would be an asset
  • Minimum two years’ experience in administrative or related field is an asset.
  • Experience working with First Nations communities in the Sioux Lookout Zone would be an asset.
  • Knowledge of the remote/isolated communities and the health priorities of the First Nations communities in the Sioux Lookout Health Authority catchment area.
  • Ability to communicate in one of the First Nations dialects of the Sioux Lookout Zone would be an asset.
  • The ability to perform the requirements of the position on a regular basis.

DUTIES & RESPONSIBILITIES:

  • Responsible processing of all intakes/referrals received for all health services by inputting of the referrals; completing intake with the client and family (if applicable) and ensure the referral and supporting documents are forwarded to the appropriate health professional.
  • Enter data into EMR (referrals, contact notes, assessments, medical reports, etc).
  • Consult on referrals with appropriate clinician.
  • Follow-up on referrals received.
  • Display a good understanding and awareness of current issues within Northern First Nation communities.
  • Knowledge of SLFNHA available resources to direct service requests and or referrals.
  • Ability to respond and act appropriately in crisis situations.
  • Ability to communicate effectively with physicians, local agencies and community-based resources.
  • Liaise with clinical supervisor and clinicians.
  • Participate in case reviews.
  • Participate in multi-disciplinary health team activities/case conferences as needed
  • Participate as part of the multi-disciplinary team on clinic days if intake is required
  • Compile Summary Reports of intake services
  • Prepare and assist with Statistical reporting, Ministry reports and Client data reports.
  • Participate in ongoing education, meetings, training, consultation with supervisor.
  • Reviewing and redirecting organizational documentation.
  • Assisting in administrative tasks
  • Coverage of reception - reception duties when required
  • And any other duties as required.
