Job Description - Program Coordinator

Position Summary 4 0hrs a week

Reporting to the Program Director, the Program Coordinator is responsible for managing and administering designated service areas of program support for individuals with disabilities.

The Program Coordinator oversees staffing and supervision, planning, budgeting and financial management, program development and implementation for the area responsible for.

The Program Coordinator ensures that all individual support is carried out with the best interest of the Individual at the forefront and in an ethically responsible manner.

The Program Coordinator will work effectively in a collaborative, team-orientated environment with other program coordinators.

Key aspects of the role will include but are not limited to the development and implementation of process guidelines, service monitoring and evaluation, service delivery coordination and intervention, budget administration and management, funding proposal development, and community liaison. Specific duties and responsibilities such as Admission and Discharge, Project development, Career and Employment Coordination, Planning Facilitation, Maintenance etc. and caseloads will be designated to each Program Coordinator. Supplemental Job Descriptions for various responsibilities will be attached to this job description for each Program Coordinator as assigned.

1. Specific Duties and Responsibilities

a. Staffing and Supervision

  • Leading and coordinating staff/human resource planning relative to particular service area that sets clear and specific job responsibilities and is in harmony with the overall staffing model of the association.
  • Providing leadership and support to Team Leaders to facilitate effective staff development and service delivery.
  • Ensure staff performance is adequately monitored and regularly evaluated.
  • Promote open communication and team building.
  • Encourage the ongoing development and growth of staff.
  • Act as a resource in regards to conflict management for association staff and families/guardians.
  • Monitor the development and implementation of staff training plans in accordance with identified staff training needs.
  • Delegate authority as necessary and clearly outline responsibilities.

b. Planning

  • Leading in the development of service delivery plans relative to a particular service area
  • Assist staff in understanding the planning process and promote their involvement as appropriate.
  • Ensure services are adequately monitored and evaluated on an ongoing basis.

c. Budgeting and Financial Management

  • Annually, and in consultation with the Program Director and Executive Director, provide program projections based on planning priorities for the program area(s) of responsibility.
  • Assist in negotiating contracts as requested by the, Program Director and Executive Director.
  • Monitor service area budgets and assist in the development of contingency plans as necessary.
  • Ensure financial policies and procedures are implemented adequately and ethically as applicable and provide input on policy and procedure revisions.
  • Monitor invoicing and financial management procedures and processes to maintain consistency and accuracy.
  • Ensure funding and service delivery proposals are developed and monitored

d. Liaison

  • Ensure and maintain effective communication with parents/guardians/family members of individuals.
  • Promote working relationships with all stakeholders.
  • Represent the organization on community committees and at community functions in order to promote community awareness of Blue Heron Support Services and its purpose.
  • Assist in the development of effective promotional and publicity materials.
  • Represent the organization regionally and provincially as requested.

e. Teamwork

  • Attend all team meetings, sessions and general staff meetings.
  • Work effectively with team members in a positive and professional manner.
  • Act as a resource for information necessary to the team in the support of the individual.

f. Individual Support

  • Ensure quality of service for individuals through effective communication with stakeholders.
  • Be available to individuals/parents/guardians for informal contact, for discussion of concerns or appreciation and for informal or formal grievance.
  • Ensure service evaluation criteria are developed and communicated effectively, and that services are monitored, evaluated and managed in a manner that optimizes opportunities for the individuals.

g. Admission/Discharge

  • Working with the Program Director and Program Coordinator assigned to

Admission/Discharge, monitor and determine availability of service for new and internal applicants for assigned service areas.
