Community Health Workers (CHWs) support clients in the community to live independently in their own home. CHWs work with clients who require assistance with daily activities including bathing, dressing, personal care, mobility and meal preparation. Without the support of the CHW, these individuals may no longer be able to live in their own home.

This position currently qualifies under the $3.00 per hour wage enhancement

A Day in the Life of A Community Health Worker- With an assigned client caseload, the CHW travels to clients’ home to provide the required assistance.

  • Personal care - Assisting with bathing, showering, toileting
  • Dressing and putting on shoes, brushing hair, teeth, shaving
  • Assisting with use of walkers and canes, transfers from bed, chair, wheelchair, toilet
  • Meal planning, safe food preparation and storage, feeding
  • Basic home maintenance and housekeeping
  • Socializing and companionship for those in need of emotional support
  • CHWs may see up to 10-12 clients in a day

Job Qualification

You will also need to (have)- Access to a reliable vehicle and a valid driver’s license (G or G2)
