Membership, in good standing, with the applicable regulatory body:College of Nurses of Ontario (RN only at HCCSS MH)College of Physiotherapists of OntarioCollege of Occupational Therapists of OntarioOntario College of Social Workers and Social Service WorkersCollege of Audiologists and Speech Language Pathologists of Ontario2+ years of recent experience in community health or a related fieldKnowledge of the health care delivery system and community resourcesExcellent interpersonal, communication, assessment, problem-solving, and decision-making skillsEffective time management, prioritization and organizational skills, with the ability to work independently and co-operatively in a busy multidisciplinary environmentEstablished ability to accurately complete required documentation, reports and formsA valid driver’s licence and access to a reliable vehicleProficient in a Windows environmentWe have a mandatory COVID-19 vaccination policy.
