Licensed Practical Nurse, Outreach

Detailed Overview Reporting to the Program Coordinator or designate and under the direction of the team leader, the Licensed Practical Nurse (LPN) Outreach works as a member of the multidisciplinary mental health services team in a collaborative nursing model with RNs and RPNs. The LPN Outreach performs assessments and plans, implements and provides personal and nursing care services to clients, both in their homes and in the community, who have a variety of mental health disorders. The LPN Outreach operates in accordance with the competency guidelines and full scope of practice within the Standards of Practice as outlined by the British Columbia College of Nurses and Midwives (BCCNM) and in conformance with established policies, procedures, standards and philosophies of Fraser Health. Responsibilities Administers oral, subcutaneous and intramuscular medications in accordance with pharmaceutical and organizational policies and procedures. Assesses and monitors the patient's actual and potential strengths and limitations including physiological, psychological, socio-cultural and spiritual needs on an ongoing basis. Provides a supportive environment for the client; contributes to the development and modification of client's plan of care; attends and provides input into interdisciplinary care planning meetings. Plans, organizes and establishes client care plan priorities when providing care; responds to unexpected events and changing client needs within scope of practice and competency guidelines. Provides nursing care to clients, both in their homes and in the community, according to identified policies and procedures, technology and Fraser Health standards; evaluates outcomes, records observations and reports problems and/or changes to designated staff. Documents and updates relevant care and procedures information to ensure patient safety. Assists clients with activities of daily living such as serving meals and feeding, client transports, bathing, dressing, skin care, toileting needs; oversees client exercise routines, as applicable. Participates in quality improvement and program activities to maintain consistent level of care and to ensure that interdisciplinary team members work collaboratively. Maintains a safe working environment for the patient and others by adhering to established safety policies; reports unsafe conditions and/or malfunctioning equipment. Assists in the orientation and education of new staff, students and other members of the interdisciplinary team, in collaboration with the Manager or designate. Performs other related duties as assigned. Qualifications Education and Experience Graduation from a recognized program of Practical Nursing or an equivalent combination of education, training and experience. Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Current First Aid, CPR, and Food Safe certificates. Completion of or eligible to complete MHSU Competency Education. Valid BC Driver's License and access to a personal vehicle with appropriate insurance coverage. Ability to observe and recognize changes in patient condition.
