A division of the Population Health and Value Based Health Systems portfolio, the Value Based Care team is focused on several areas of work as part of Ontario Health’s approach to improving population health and creating greater value for patients and caregivers. These areas include funding strategies that better reflect the nature of patient conditions across their continuum of care, data and analytic products to support Ontario Health Teams (OHTs) and management of population health, performance and accountability development for OHTs, and value-based strategies to improve the delivery of integrated care. The Strategy Lead, Value Based Programs, will work closely with internal clients and external stakeholders to identify and set strategic priorities and ensure that value-based care and population health objectives are continually integrated into portfolio plans and deliverables. The Strategy Lead should be a strategically minded candidate, with a strong understanding of the health system, a demonstrated ability to see the bigger picture, and who can provide strong thought leadership amidst diverse and complex topics. In collaboration with health system partners and teams across Ontario Health, the key focus areas for this role will include:Supporting the development of the strategic plan/ opportunities within the division through the development, implementation and maintenance of a strategic roadmapSupporting value-based and integrated care program implementation, including leading initiatives that advance integrated and value-based care maturity and performance of Ontario Health TeamsStrategic and financial impact analysis of value-based and integrated care planning and funding opportunitiesHere is what you will be doing:Leads research and analysis, inclusive of scans, market analysis, and comparative policy assessments, to support integrated care strategy, business planning and decision-makingLeads strategic partnership initiatives with OH’s system partners ensuring that value-based care and population health principles are embedded in new policies and funding models to advance integrated careWorks with senior level clinicians and administrators to design and enable the implementation of programs that focus on better addressing the continuum of care for patientsProvides consultative and analytical support regarding interpretation and presentation of both program models and data, preparing ad hoc reports and presentations to assist with projects and decision-makingWorks across Ontario Health portfolios, the Ministry of Health, OHTs and other healthcare providers in a collaborative and consultative manner to advance value-based and integrated care initiativesApplies a deep understanding of health system policy initiatives and funding reform instruments to drive OH’s strategic positioning and partnership opportunities with respect to value-based and integrated careLead the development of proposals for new strategies, projects and funding that align with Ministry of Health policies and Ontario Health’s corporate strategyKeeps abreast of value-based care and population health trends, finding innovative and creative ways to incorporate key learnings and effectively communicating these to OH teams and system partners to support system transformationCultivates innovative system thinking and planningWorks collaboratively with other team members in advancing larger divisional objectives, including Ontario Health Team prioritiesPerforms other related duties as assignedHere is what you will need to be successful: Education & Experience Undergraduate degree in the Health Sciences, Business, Public Policy, Economics or other relevant field; graduate degree (e.g. MHSc, MHA, MPA, MHI) is preferred5-7 years’ experience in roles focused on health system planning, health policy and/or program deliveryThorough knowledge of Ontario’s health system and experience working with a wide range of clinical and administrative stakeholdersDemonstrated experience in health system planning, with an understanding of how to strategize and/or implement value-based programs and methodologies that follow a patient’s continuum of careKnowledge of, and experience with, government policy development and approval processes would be strongly advantageous; broad management consulting experience and strategy development/execution experience would be an acceptable alternativeStrong grasp of health policy, and health system funding reform instruments and other payment/ funding modelsFamiliarity with procurement and contracting norms in Ontario would be highly advantageousAwareness of emerging IT trends and their impact on healthcare deliverySkills and Abilities​Demonstrated ability to establish strong relationships within an organization, collaborate effectively and maintain outstanding service delivery standardsAbility to think strategically, ask guiding questions and understand the strategic significance of data/resultsExceptional writing skills, along with an ability to present solutions and results from complex analysis in a clear and compelling mannerSuperior analytical skills, including data analysis and interpretation, especially in support of strategic planning and decision-makingExceptional problem-solving capabilities that can be applied within a complex and dynamic environmentProven ability to work as part of a high-performing team, with a results-driven focusFlexibility in taking on evolving work in a rapidly changing environment, relative to priorities at handEmployment Type: Permanent Full TimeSalary Band: 6Location: GTA, Ontario (currently hybrid; subject to change).
