SUMMARY

Reporting to the facility director, the SW is part of a strong interdisciplinary, collaborative clinical team that offers support in the field of addictions. He/she is in charge of creating, maintaining and strengthening our link with other social service providers, the family and community.

RESPONSIBILITIES

  • Participate in a combination of social services according to the clientele’s needs
  • Support intervention plans in accordance with a community approach
  • Cooperate, with the multidisciplinary team, to the creation, realization and evaluation of programs and intervention or rehabilitation plans
  • Involve the residents, their family members and community in the implementation of intervention plans
  • Refer the residents to community resources available for his needs
  • Preparation and presentation of case conferences
  • Participate as a member of the admission committee
  • Offer support to the groups under the supervision of the family services coordinator
  • Encourage the participation of residents in the development of activities within the program
  • Write daily reports, record the performance of registered actions and implement actions, if required
  • Collaborate with the case workers planning the intervention with the family
  • Assist the clinical team in the execution of their mandates
  • Participate in any other related task required by their director

REQUIRED PROFILE

  • Hold a BSW Degree
  • Member of the Ontario College of Social Work and Social Service Workers (mandatory)
  • Interest and experience within the field of addictions, in therapeutic community or mental health
  • Good knowledge of software programs: office suite
  • Good knowledge of laws and regulations of the MCCSS
  • Proven skill in team work
  • Planned and organized
  • Excellent communication skills
  • Bilingual is an asset
  • Any other relevant experience or studies are an asset

OTHER INFORMATION

  • Job type : permanent full-time
  • Schedule : variable schedule (37.5 hours/week) and must be available to work evenings and weekends
