Mental Health Navigator

The Aspen Primary Care Networks (APCN) is seeking a full time, permanent Mental Health Navigator. The primary function of this position is to provide mental health intake/assessment, navigation, referrals and follow-up to patients. The Mental Health Navigator will work collaboratively with the PCN multidisciplinary teams to enhance the delivery of mental health services for patients across all age groups.

This position is based in Athabasca, AB.

RESPONSIBILITIES/DUTIES :

  • Works to full scope of practice with an emphasis on providing competent, empathetic and compassionate care
  • Provides services based on highly developed knowledge and skills
  • Conducts initial mental health screening assessments, navigation of community resources, referrals, and follow-up
  • Monitors mental health waitlists and ensures comprehensive and timely follow-up
  • Collaborates with the PCNs multidisciplinary teams and community resource providers and stakeholders
  • Ensures comprehensive and up to date knowledge and understanding of community and external resources and referral pathways

COMPETENCIES, EDUCATION & EXPERIENCE :

  • Minimum 2 years mental health experience is required.
  • Experience in a primary care setting an asset
  • Mental Health First Aide, Suicide Intervention Training, and/or equivalent preferred.
  • Bachelor’s degree or diploma in a health-related field (e.g. Licensed Practical Nurse, Registered Social Worker, Registered Social Worker).

CONDITIONS OF EMPLOYMENT :

  • Proof of clean criminal record check (vulnerable sector) required
  • Completion of reference checks
  • Copy of current registration with governing body (e.g CLPNA, ACSW)
  • Copy of current CPR
  • Own vehicle with valid driver’s license
