Annual Salary: $68,308 - $85,390

Under the direct supervision of the Director of Care, the Behavioural Support Lead Facilitator is responsible for leading activities related to building knowledgeable care teams, and the implementation of evidence-based practices, research knowledge and expertise related to Behavioural Supports Ontario (BSO) into clinical practice within all care floors of Hogarth Riverview Manor.

Qualifications : Education/Experience :

  • A diploma in a relevant health science or related discipline
  • Current registration as a Registered Practical Nurse with the College of Nurses of Ontario, without restriction preferred
  • Minimum of three years experience in geriatrics, long-term care or community based seniors care
  • Experience in program development, research and evaluation, community development, and education

Skills/Abilities :

  • Ability to communicate effectively and concisely, both orally and in writing, with the ability to make presentations and deliver training to a variety of audiences
  • Ability to establish and maintain effective relationships with employees, residents, families, and community partners, ministries and other external stakeholders
  • Proven ability to function and contribute as an effective team member and to work independently
  • Proven ability to provide coaching and mentoring
  • Excellent project/time management, analytical/critical thinking, problem-solving, and decision-making skills
  • Strong leadership skills including the ability to share skills and knowledge with others
  • Must be proficient in the use of computers and current related word processing, spreadsheet, and presentation software
  • Demonstrated regular attendance at work

CONDITIONS OF EMPLOYMENT

  • Will be required to travel to various work locations and provide own transportation
  • This position shall be conditional upon a satisfactory Police Vulnerable Sector Check (PVSC) where indicated, to ensure the absence of relevant criminal convictions.
